Lakeview Elementary School

 

All students who participate on any athletic team must complete an RUSD-Sports-Packet.pdf Athletic Packet.   A physical exam is required.  Please have the doctor fill out the Sports Physical Examination Form and return it to the school office.  Packets are also available in the office.  Students will not be able to participate in practices or games until the Sports Physical Exan form has been turned in. ALL pages of the above mentioned packet must be completed.  A completed packet includes the sports physical signed by a physician, team participation, \cConcussion and heat illness forms.  The driver/vehicle forms do not need to be completed.

 

PARENT VOLUNTEERS:

Parent volunteers make everything better. We need you and we appreciate you! In order to volunteer you must have the same clearance as you need for the classroom and follow all district covid procedures.  Please check with the front office to determine your current volunteer status, or to pick up a volunteer registration form.
